The Support Advisor will work with the person and together write down what their service will look like; this will include how their team will be recruited and trained; who the person wants to attend meetings and have their information shared with; how and when the person will want to plan and review their service and how the person can make suggestions about their service or complain if they are not happy about their service or if they want to leave C-Change.
The Support Advisor will review the Support Agreement with the person every year to check whether the person wishes to change anything. The Support Advisor will also agree a ‘How I Manage My Support’ document with each of the people they work for. This will detail what aspects of their service the person will manage themselves, what things they want the Support Advisor to support them with and things they want the Support Advisor to do for them. This is reviewed each year.
