Once a person chooses C-Change to work for them a local Support Advisor and Area Leader will meet with the person and their loved ones’ to chat with them about what they want to do and how they want to be supported. The person will be supported to think about how they want to make best use of their budget and to plan the best way forward, to achieve their outcomes.
The person may want to use their budget to pay for a membership of a club, take a short break, pay for assistive technology (such as a stair climber) and purchase some direct support from team members.
How a person is choosing to spend their budget will be included in their Service Design, which describes what a person’s service will look like. This will be shared with the person and their loved ones for approval and signing off, before sharing the Service Design with the Care Manager.
